Baptism Forms

Congratulations! We look forward to welcoming your child to our faith through Baptism.

To schedule a Baptism date, first fill out the form below, and contact us to find what dates are available.

Please return all completed forms at least two weeks before the Baptism.

Baptism Godparent form - Download and print or we can print one for you. Pastor signature and seal required, each Godparent must complete a form.

Baptism Class for parents and Godparents are scheduled frequently - contact us to register for a class.

Baptisms scheduled once a month on a Saturday afternoon. Date will be scheduled after registering.
