Assumption of Risk, Medical Permission and Release, and Photography Release:

It is understood and acknowledged that injury may be sustained because of the potentially hazardous nature of Cat Chat activities. I
freely acknowledge, and willingly accept and assume risk of injury that might occur from participation in activities on the grounds and
in any facility of St. Joseph Catholic Church. If applicable, I have verified all participants of my ministry group have signed an
Assumption of Risk statement for this activity. In the event of such injury to myself or my child, I hereby give my permission and
consent for treatment deemed necessary for a condition arising during participation in related activities, including medical and surgical
treatment recommended by a medical doctor. I agree to take responsibility for payment of the usual charge for such treatment. I
release St. Joseph Catholic Church, its employees, agents, volunteers and assignees from any and all claims, causes of action suits, and
related rights for himself/herself, his/her estate, his/her heirs, administrators, executors, etc. for any and all injuries, illnesses, and
damages sustained as a result of participation in activities held by St. Joseph Catholic Church. This release applies to any present or
future injuries and it binds my heirs, executors, and administrators. If applicable, I have verified all participants of my ministry group
have signed a Medical Permission and Release for this activity.
I understand that participants may be videotaped or photographed during this activity. I release the right of those video tapes and
photographs to St. Joseph Catholic Church. I also agree that any photographic images captured of me during my use of these facilities
may be used in parish promotional materials such as the bulletin, website, brochures, posters, podcasts, videos, etc. If applicable, I
have verified all participants of my ministry group have signed a Photography Release statement.

At the Abundant site,
1. At the AMOUNT field, enter the payment amount calculated above.
2. At the FUND field, use the drop down arrow to select the Vacation Bible School category.
3. At the EMAIL ADDRESS field, enter a valid email address. After successfully completing an online payment, you will receive an email from Abundant confirming receipt of your gift.

You will receive a Wildernss Through the Sacraments Welcome email within 7 days of the completion of your registration.
